We accept all major credit cards.

Insurance: 

 

We accept nearly all major insurance plans including commercial PPOs and the BCBS HMO advantage through the Exchange.

For specific questions about your policy, please contact your insurance provider.

 

We do not accept Medicaid, Molina, Ambetter, Amerigroup, CHIP, or STAR.

Additional Fees:

A no-show fee for missed appointments is $75.00 for new patients, annual physicals, and office procedures. For all other visits the no-show fee is $50.00.

 

A returned check fee is $35.00. 

 

There is an additional $35.00 dollar fee for completion of forms for use outside of the patient's medical record. This includes:

  • Family Medical Leave of Absence Forms 

  • Medical Records: The first patient copy is free of charge, additional copies are $25.00
