If you are establishing or currently receiving care at Sparks Clinic and want to ensure the smoothest continuity of care from your previous or external care provider(s), you may bring your medical records to your visit and staff will add them to your permanent record.
Patients have the right to request their medical records at any time. Your first copy is complementary. Subsequent copies are available for a $25.00 fee.
HIPAA Form:
The Health Insurance Portability and Accountability Act of 1996 requires that all patients complete this form before any medical records are sent or received. In the case that you need medical records transferred to or from our office to another facility, please complete these forms and return them to the front office.
